Outreach Stories
The City is proud of our hardworking staff members who are dedicated to helping those most in need in our community. While some days might be more difficult than others, there have been many moments where staff has been able to help the most vulnerable members of our community by making contact with them and connecting them to much-needed services. Below is a collection of Outreach Stories and updates about the help our staff has been able to provide and about those who are sometimes invisible in our community.
MARCH 2024
- While conducting outreach in the City of Stanton, Outreach Coordinators engaged with a middle-aged female individual experiencing chronic homelessness. The individual was provided a form of housing documents required to be entered into the Coordinated Entry System. An appointment was set up for the female to be transported by the Outreach Coordinators to Orange County Clerk Recorder, to obtain her Birth Certificate to start the Housing Navigation process. After gathering all documents by the end of the week a profile was created and assessment was completed for the female to be successfully on the North SPA housing match opportunities.
- The Outreach Coordinator received a call from an individual experiencing homelessness. The individual was previously engaged with the prior outreach team and wanted to reconnect to services. The individual was seeking shelter and multiple requests were made to obtain a bed at the Buena Park Navigation Center. After multiple attempts, a bed was successfully obtained. Outreach coordinators provided transportation to the shelter. The Outreach Coordinator met with the operator at Mercy House to conduct a warm hand-off.
FEBRUARY 2024
- Stanton Outreach Coordinators engaged with a male individual facing chronic homelessness who needed medical care. The Outreach Coordinators connected and scheduled the individual an appointment with his primary care physician. The individual was transported to his doctor’s appointment. He was provided physical care and prescribed a prescription from a local Pharmacy which has been helping with his medical condition. Since then, the individual has continued receiving medical care from his physician and is accepting some additional treatment. This individual is working on the housing navigation process and currently waiting for a Permanent Supportive Housing opportunity.
- Working collaboratively with the HOPE Center, Stanton Outreach coordinators have been assisting an individual in obtaining all her documents to add her to the community queue for housing opportunities. HOPE Center staff scheduled a notary appointment for the individual’s birth certificate and unfortunately, they were unable to transport her. Stanton Outreach stepped in and Ubered the individual to her appointment. Since then, outreach coordinators have ensured that the individual has transportation to all her appointments including her mental health assessment with the County of Orange.
JANUARY 2024
- A woman experiencing street-level homelessness had been previously connected to mental health services and lost contact and needed help getting reconnected to mental health services. Stanton’s Outreach team was successfully able to arrange her an appointment for that same week to start the assessment process for mental health services. Outreach Coordinators provided additional help with the collaboration of our Code Enforcement department helping to store the participant’s belongings to make sure she made it to her appointment. The participant has been able to follow through with her next appointment.
- A homeless individual, who has struggled with the continuation of homeless services reached out to the Outreach Coordinators for assistance. The individual acknowledged their substance dependency and wanted help to combat their substance use. The Outreach Coordinators assisted the participant with getting connected to Substance Abuse treatment by contacting a local rehab center to obtain a bed for the individual, an assessment was done over the phone, and it was determined that he needed to attend to his open wounds before being admitted to Substance Abuse treatment. The Outreach Coordinators will be communicating with the individual to guide him through the process.
DECEMBER 2023
- Outreach Coordinators received a call from a homeless couple who have been engaged with staff and have received services in the past. The couple has been experiencing homelessness and seeking shelter resources. The Outreach Coordinators assisted the individuals by submitting a referral for a couple’s bed to the Buena Park Navigation Center. The couple’s bed was obtained, and the coordinators coordinated transportation services to the shelter in Buena Park.
- Outreach Coordinators successfully placed two longtime homeless individuals who were formerly on the streets of Stanton. Outreach coordinators completed the process of getting the individuals placed on the Individual Coordinated Entry System (ICES) housing list by guiding them through the difficult process of obtaining all the necessary paperwork and certifications. Once this part of the process was complete the individuals were placed on the list for housing opportunities and provided with regular assistance and continued to check in weekly, sometimes daily contact with the Outreach Coordinators. The individuals were placed in Clara Vista as part of Project Homekey, they are the first two placements that had ties to the City of Stanton. The Outreach Coordinators will remain in contact with both Clara Vista residents and continue to support them moving forward.
NOVEMBER 2023
- A participant who is engaged with our Outreach Coordinators was seeking assistance in locating a shelter. The Outreach Coordinator was able to confirm the participant's ties to Stanton. The Outreach Coordinator reached out to the Placentia Navigation Center to obtain a bed for the individual at their shelter. Transportation was provided to the Placentia Navigation Center. The Outreach Coordinators met with the operator at PATH to conduct a warm hand-off.
- Outreach Coordinators are working towards housing with an individual who has strong Stanton ties. Outreach Coordinators were able to obtain a disability appointment from Social Services to see if he qualified for disability. Transportation was provided to his appointment and the participant is awaiting correspondence from social services.
OCTOBER 2023
- Outreach Coordinators engaged an individual who had previously received services from multiple agencies but lost contact with her case manager. The participant stated that she did not want to be reconnected with former agencies and preferred to work with the Outreach Coordinators in the process of pursuing permanent supportive housing. The participant was also interested in getting connected to Mental Health resources, an intake appointment was scheduled with the assistance of the Outreach Coordinators, and the participant was transported to her appointment with OC Health Care Agency.
- A couple from the homeless community who have struggled with the continuation of homeless services reached out to the Outreach Coordinators for assistance. The couple both wanted shelter. The Outreach Coordinators requested a couple's bed at the Buena Park Navigation Center. Outreach Coordinators were successful in obtaining a couple’s bed. The participants were transported to the shelter and will now receive assistance from Mercy House to help them navigate the individual coordinated entry system.
SEPTEMBER 2023
- An individual who had lost his job contacted Stanton’s Outreach Coordinators for assistance. The individual requested help with family reunification, as he is originally from Pennsylvania. Stanton’s Outreach Coordinators contacted the participant’s family member to confirm that the individual would have a place to stay upon arrival. After confirming housing placement, Stanton’s Outreach Coordinators proceeded with the family reunification process through Greyhound transportation.
- Stanton’s Outreach coordinators currently have two individuals, who were recently matched to a housing opportunity at Clara Vista also known as the Tahiti. Both individuals are currently engaged with the outreach team and are both working towards ensuring all their documents are properly filled out and returned to Orange County’s Housing Authority. Both clients are expected to be moved in by the end of November 2023.
AUGUST 2023
- Stanton Outreach Coordinators engaged with a middle-aged female individual who was interested in receiving services and resources. After speaking with the individual, she mentioned she was interested in Mental Health Services. The individual was also interested in Shelter, however, she wanted to prioritize her mental health prior to going into a shelter facility. Outreach Coordinators were successful in getting an appointment date for the individual, she ensured to attend her appointment. Stanton Outreach Coordinators provided her with a bus pass and a reminder call to ensure she did not miss her intake assessment appointment in the City of Westminster.
- Stanton’s Outreach Coordinators engaged an individual who had previously received services from various agencies but lost contact. The participant stated that he did not want to be reconnected with former agencies but instead wanted to work with the Outreach Coordinators to pursue permanent supportive housing. The Outreach Coordinators worked with the participant to obtain the required documentation needed for a permanent supportive housing application. As a result, the participant was successfully matched to a housing voucher through the Orange County Coordinated Entry System. The Outreach Coordinators are currently submitting final documentation.
JULY 2023
- While performing street outreach, Stanton Outreach Coordinators engaged a male individual who was recently experiencing street-level homelessness. The individual expressed not feeling well, medical attention was provided right away a call was made to Orange County Fire Authority and Paramedics, and he was then transported to a local hospital in Orange County. He was referred to Costa Mesa College Hospital for detoxication and mental health treatment. He will be treated for some days there and assigned a case manager to help him with further interim housing options. Residential psychiatric programs would be an option for him to accept. Programs like these will provide a continuum of patient-focused services for ongoing stabilization.
- Outreach Coordinators received a call for service from the staff at the community services counter. They engaged with an individual who had recently become homeless due to losing his job and apartment. The individual had been participating in the senior lunch program at the community center and reached out for services. The individual was run for ties and verified by our Orange County Sheriff’s Homeless Liaison Deputy. A male bed referral was requested at the Buena Park Navigation Center, after a couple of attempts he was finally accepted and transported. While at the Buena Park Navigation Center, the individual will be assigned a case manager and work towards permanent supportive housing. While in the shelter the individual continues to shuttle to our senior lunch program daily.
JUNE 2023
- Outreach Coordinators engaged with a middle-aged individual experiencing homelessness and in need of services. The individual had just recently became homeless after being evicted from his apartment in Orange County. After engaging the individual and discussing services and resources he would be connected to, he was interested in getting into shelter. After verifying ties with proof of Identification and checking-in daily for a male bed reservation. Buena Park Navigation Center accepted the bed referral and later that same day he waited at one of the shelter’s shuttles stop and was successfully transported to the Buena Park Navigation Center. The individual was very grateful and thankful to be indoors. There he will receive ongoing case management and housing navigation.
- Outreach Coordinators have been engaged with a couple experiencing homelessness, who had been sleeping in their vehicle unfortunately, they were involved in a terrible car accident and their vehicle was totaled. They were seeking a couple’s bed at the Buena Park Navigation Center. Multiple bed reservations were requested and denied due to availability. After approximately a week of requesting a couple's bed they were finally matched. The participants will now receive assistance from Mercy House to help them navigate the homeless service system.
MAY 2023
- Stanton's Outreach Coordinators received a referral from the Buena Park Outreach team for an elderly female who had strong ties to the City of Stanton and the City of Buena Park and had recently been experiencing homelessness and was seeking shelter. The outreach coordinators assisted the individual by submitting multiple shelter referrals to the Buena Park Navigation Center. With the assistance of the Buena Park Outreach team, the Stanton outreach team was able to secure a bed for the individual. Outreach coordinators coordinated transportation services to the Buena Park shelter. At the Buena Park Navigation Center, the individual will receive supportive services that include housing and case management.
- Stanton's Outreach Coordinators received a call from a Community Services staff to engage an elderly male in need of services. The elderly male had been experiencing homelessness and seeking shelter resources. The Outreach Coordinators assisted the individual by submitting shelter referrals to the Buena Park Navigation Center and the Placentia Navigation Center. A bed became available, at the Placentia Navigation Center. The Stanton Outreach team coordinated transportation services to the shelter in Placentia and placed the participant in shelter that same day.
APRIL 2023
- Outreach Coordinators have developed a relationship with a member of the homeless community that has been on the streets of Stanton for more than 7 years. The 50-year-old male has struggled with various forms of dependency but has always been cooperative when dealing with City staff. As a result of a collaborative effort involving multiple agencies, Stanton’s Outreach Coordinators have facilitated the individual’s eligibility for permanent housing. Included in this process have been scheduling and transportation to mental health appointments, court dates and various appointments related to addiction treatment. He is currently housed using relocation assistance funding and while he awaits permanent housing.
- While conducting outreach, Outreach Coordinators contacted an elderly female individual located at Martha Weishaupt Park. The individual indicated that she was currently unhoused and in need of services. Staff created a profile in Outreach Grid due to the individual being from out of the area. During this process, it became clear that the individual needed medical attention. The Orange County Public Health Nurse was contacted to conduct further evaluation and determined that she met the requirements to be transported to a Covid isolation site operated by The Illuminate Foundation. At this location she will receive medical treatment as well as housing assistance.
MARCH 2023
- While responding to a call for assistance from the Orange County Sheriff's Department, the Outreach Coordinators engaged an older man waiting outside the Sheriff's Substation. The individual requested shelter and housing services. While a long-term bed became available Stanton Outreach Coordinators transported the individual to the Cold Weather Emergency Shelter. After, a few nights of staying at the temporary shelter, he was seeking a long-term stay. Transportation was provided to the individual and he is now at Placentia Navigation Center. The client will have support services that include housing, case management, and employment.
- Stanton's Outreach Coordinators received a call from a client who has been experiencing homelessness and seeking shelter resources. The Outreach Coordinators assisted the individual by submitting a shelter referral to the Buena Park Navigation Center. The Stanton Outreach team coordinated transportation services to the shelter in Buena Park. At the Buena Park Navigation Center, the client will receive supportive services that include housing and case management.

- Stanton’s Outreach Coordinators received a self-referral call from the hotline from a previous Stanton resident seeking services for herself. The participant had called to inquire about the type of assistance she could obtain because she no longer had a stable place to stay. Outreach Coordinators engaged with the participant and concluded that the participant was not living in a site meant for human habitation. After further evaluation, he was deemed eligible for homeless service assistance. As a result, the participant was taken to the County operated Cold Weather Shelter, where he will continue to work with the shelter provider PATH for further housing services.
- Stanton's Outreach Coordinators received a call for service from Library Staff. Upon arrival, Stanton's Outreach coordinators engaged a male seeking medical aid and shelter. The Outreach Coordinator connected the participant to the Orange County Public Health Nurse for further evaluation. Then, the Public Health Nurse deemed the participant appropriate for a covid isolation site run by Illumination Foundation. The participant was taken to the isolation site and will receive further medical support and linkages to housing services.
- OCSD came across a female experiencing homelessness. The female
frequented both Stanton and Garden Grove. The participant expressed that she did not have anywhere to go or any family support. The OCSD and Outreach Coordinator were able to work and collaborate to transport and obtain a shelter bed at Buena Park Navigation Center. The participant will now continue to work with the shelter operator, Mercy House, for further support services.

- The Outreach Coordinators received a self-referral on the hotline from a previous Stanton resident seeking services. The caller indicated she was in an unstable housing situation and inquired about available services. Outreach Coordinators engaged with the female participant and concluded that her place of temporary residence was not meant for human habitation. After further evaluation, she was transported to the Buena Park Navigation Center where she will work with Mercy House for further housing options.
- Based on a referral from City staff, the Outreach Coordinators engaged a female seeking medical aid and shelter. The participant was connected to the Orange County Public Health Nurse for further evaluation and treatment. The Public Health Nurse determined that the participant was eligible to be housed at a Covid isolation site run by the Illumination Foundation.

- While offering case management to a client, the Outreach Coordinators informed the 74-year-old male client that he had been matched to a county housing subsidy. Outreach Coordinators provided transportation for the client to Project Homekey in Anaheim. While at Anaheim’s Project Homekey, the client will receive supportive services such as long-term shelter, mental health or addiction treatment, job placement, basic life skills and educational opportunities. The Outreach Coordinator will offer periodic check-ins with the client.
- While responding to a call for assistance from a concerned resident, the Outreach Coordinators engaged a female participant who has been experiencing chronic homelessness. Based on the referral, the Outreach Coordinators began the process of connecting the participant to County housing resources. The Outreach Coordinators completed a housing assessment and referred the participant into the queue County’s Coordinated Entry System. Once the participant was evaluated, she was deemed appropriate for placement at Anaheim’s Project Homekey. This collaboration has led the participant to receive additional supportive services that include housing, case management, and medical care.
OCTOBER 2022
- During a collaborative street outreach effort, Orange County Health Care Agency and Stanton's Outreach Coordinators encountered a male in need of minor medical attention experiencing homelessness. Assessing the situation, the Outreach Coordinators contacted Cal-Optima to request a Mobile Care Unit (MCU). Cal-Optima's MCU arrived on scene to provide medical assistance to the male participant. Following the medical treatment, the participant expressed that they required assistance contacting their housing navigator. The Outreach Coordinators contacted People Assisting the Homeless, a community partner, to coordinate a meeting regarding the participants housing situation.
- While offering case management to a client, the Outreach Coordinators informed the 65-year-old male client that he had been matched to a county housing subsidy. Outreach Coordinators provided transportation for the client to Oasis Homekey in Huntington Beach. While at Huntington Beach’s Oasis Homekey, the client will receive supportive services such as long-term shelter, mental health or addiction treatment, job placement, basic life skills and educational opportunities. The Outreach Coordinator will offer periodic check-ins with the client.
- While responding to a call for assistance from a concerned resident, the Outreach Coordinators engaged an elderly male participant who expressed the need for medical attention. The Outreach Coordinators connected the participant to a mobile medical response unit. Once the participant was evaluated and treated, he was deemed appropriate for placement at the Buena Park Navigation Center. Additionally, the Outreach Coordinators completed a housing assessment and referred the elderly participant to the county community queue for Orange County-funded housing. As a result, the participant was able to obtain supportive services that included housing, case management, and medical care.
SEPTEMBER 2022
- While performing street outreach, Stanton Outreach Coordinators engaged an elderly man experiencing street-level homelessness while in need of medical attention. Working collaboratively with Cal-Optima, the Stanton's Outreach team was successfully able to arrange a field nurse to respond to the location of the participant. Nursing personnel from Families Together, an organization providing basic medical attention to those who are most vulnerable, was able to connect the participant addition medical resources. After being treated, the participant expressed that he had been renting a room in the City of Buena Park and had been evicted. He expressed interest in obtaining interim housing and worked with the Outreach Coordinators to determine the best next step. The individual was able to demonstrate ties to North Orange County and was eligible for placement in the Buena Park facility. Outreach Coordinators contacted Buena Park Navigation Center for bed availability. That same day Stanton’s Outreach Coordinators were able to obtain a male bed and provided transport for a successful stay at Buena Park Navigation Center.
- While responding to a call for service from OCSD, the Outreach Team encountered a family passing through the city that had simply run out of gas. This family did not have the means to obtain more gas and was in the process of setting up in the city. The Outreach Team was able to provide the funding for gas needed to get to their destination as well as provide a meal for the children of the family.
AUGUST 2022
- The Outreach Coordinators engaged an elderly woman experiencing homelessness on the streets of Stanton. The individual was interested in medical detox for alcohol dependency and inquired if she could be referred into a program and receive this service. The Outreach Coordinators collaborated with the treatment team, “The Anaheim Lighthouse,” which provided connectivity with a licensed detox facility. Additionally, the Outreach Coordinators provided a pickup location and waited with the individual to conduct a warm hand-off with the receiving team.
- While responding to a request for service from concerned residents, the Outreach Coordinators engaged a male individual who was unconscious and in a medical gown roaming Stanton. Upon arrival, Stanton’s Outreach Coordinators accessed the situation to best arrange services for the individual. The Outreach Coordinators successfully contacted the individual’s caregiver to coordinate reunification. Collaborating with the Orange County Sheriff’s Department, transportation services were provided to the male individual to Anaheim West Medical. The individual received medical attention to open wounds and was appropriately discharged.
- While performing street outreach, Stanton’s Outreach Coordinators engaged a female experiencing street-level homelessness while in need of medical attention. Collaborating with community partners, the Outreach Coordinators were successfully able to connect the female participant to the on-site medical care. Families Together provided the female participant with wound care and reconnected her to TeleCare for mental and behavioral health services. The individual was provided with information regarding the resources available to individuals experiencing homelessness.
JULY 2022
- While responding to a call for assistance from the Orange County Sheriff's Department, the Outreach Coordinators engaged with an elderly female individual who has been experiencing chronic homelessness. The individual expressed an interest in receiving shelter and housing resources. The Outreach Coordinators assisted the individual by submitting a shelter referral to the Buena Park Navigation Center. The Stanton Outreach Coordinators coordinated transportation services to the shelter in Buena Park. At the Buena Park Navigation Center, the client will have supportive services that include housing, case management, and medical care.
- Stanton Outreach Coordinators received a call from a transitional-aged youth (18 to 26 years old) that expressed an interested in getting referred into a shelter to prevent falling into street-level homelessness within the city. Working alongside the Orange County Sheriff's Department, Stanton's Behavioral Health Deputy was able to confirm the individual's ties to the North Service Planning Area. Once ties were confirmed, the Outreach Coordinators collaborated with Placentia Navigation Center to connect the participant to shelter and additional resources. The Outreach Coordinators assisted the individual with transportation to Placentia Navigation Center to conduct a warm hand-off with the staff and the participant was awarded a Housing Choice Voucher on 7/1/22.
- Responding to a request for service from concerned residents, the Outreach Coordinators engaged a male individual roaming Stanton in a medical gown. The individual would slip in and out of consciousness during contacts with staff and was clearly a danger to himself. Stanton’s Outreach Coordinators arrived on scene and accessed the situation to best organize services for the individual. The Outreach Coordinators were successfully able to contact the individual’s caregiver to coordinate reunification. Collaborating with the Orange County Sheriff’s Department, transportation services were provided to the male individual to Anaheim West Medical. The individual received medical attention to open wounds and was connected with his caregiver to continue his recovery.
- While performing street outreach, Stanton’s Outreach Coordinators engaged a female experiencing street-level homelessness in need of medical attention. Collaborating with community partners, CalOptima’s Mobile Care Unit and Families Together, the Outreach Coordinators were successfully able to connect the female participant to the appropriate medical care. Upon arrival, Families Together provided the female participant with wound care and reconnected her to TeleCare for mental and behavioral health services. Additionally, the participant was informed of the plethora of medical benefits that are available to members of the homeless community. The Outreach Coordinators will continue to collaborate with community partners to provide connectivity to medical assistance to those in need.
JUNE 2022
- Stanton Outreach Coordinators engaged a newly homeless middle-aged male that needed shelter and supportive services. As a result of maintaining regular contact with the individual throughout the course of one-month, he was located when a bed did become available. The individual was connected to a shelter and will continue to receive additional supportive services on-site from a case manager. Additionally, the Outreach Coordinators provided the individual with transportation to the shelter and conducted a warm hand-off with the staff of Bridges at Kraemer Place.
- Stanton Outreach Coordinators engaged an elderly individual in need of medical care and facing chronic homelessness. The Outreach Coordinators connected the individual to Serve the People and coordinated the response of their mobile medical unit. The individual received physical care and was able to receive a prescription from a local pharmacy. The induvial was in such need of continued medical care that were admitted in a recuperative care facility to facilitate additional treatment.
- Stanton Outreach Coordinators established and maintained contact with a transitional-aged participant (18-26 years old). The individual was an Orange County resident who had recently become homeless and was in dire need of housing assistance. The Outreach Coordinators collaborated with the Placentia Navigation Center to place the participant in the shelter and connect the individual with additional resources.
- While responding to a call for assistance from a concerned resident, the Outreach Coordinators engaged an elderly couple who has been experiencing chronic homelessness. Based on the referral, the Outreach Coordinators began the process of connecting the participant to County housing resources. Once the participant was evaluated, the couple was deemed appropriate for placement at the Buena Park Navigation Center. The Outreach Coordinators also completed a housing assessment and referred the couple into the County community queue for County-funded housing. As a result, the couple could obtain supportive services that include housing, case management and medical care.
- Over the past month, the Outreach Coordinators have responded to calls for assistance from concerned residents residing near Garden Grove Blvd. and Beach Blvd. The increase in the calls comes from the encampment clean-ups that have been occurring in Garden Grove, Westminster, and under the 22 Freeway. The Outreach Team has been focusing heavily on this area to ensure individuals do not set up encampments and assist individuals with homeless service navigation and connectivity to shelter in their original place of origin.
MAY 2022
- Stanton’s Outreach Coordinators received a referral from a neighboring City regarding a Stanton family experiencing street-based homelessness in the Central Service Planning Area. An elderly mother and daughter have been residing in their vehicle over the course of a year. The Outreach Coordinators were successfully able to relocate the family back to Stanton allowing for supportive services to be coordinated. Additionally, the Outreach Coordinators will be providing case management to the family while connectivity is established with Family Services for a warm handoff.
- Outreach Coordinators engaged a couple in need of supportive services and looking to connect to a healthcare provider. After assessing the situation, Stanton’s Outreach Coordinators reconnected the couple to a service provider within the Central Service Planning Area where they had been receiving case management services and exploring housing options. Additionally, the Outreach Coordinators were able to collaborate with Cal Optima‘s Mobile Care Unit (MCU) providing basic wound care to the couple. Cal Optima’s MCU dispatches a Field Nurse to vulnerable individuals for basic health treatment and can assist with coordinating future health appointments. - While responding to a call for assistance from a concerned resident, the Outreach Coordinators engaged a female participant who had been experiencing chronic homelessness. Based on the referral, the Outreach Coordinators began connecting the participant to County health resources. Once the participant was evaluated, she was deemed appropriate for placement at a county health treatment center. Equally important, the Outreach Coordinators completed a housing assessment and referred the participant to Orange County-funded housing for placement on the list to receive assistance. As a result, the participant obtained supportive services that include housing, case management, and ongoing medical care.
- While responding to a call for assistance from a concerned resident, the Outreach Coordinators engaged a middle-aged male individual experiencing chronic homelessness. The individual was exposed to a confirmed COVID-19 case and was experiencing COVID-19 symptoms. Based on the information, the Outreach Coordinators connected the participant to a County funded COVID-19 isolation site. Once the participant was evaluated, he was deemed appropriate for placement at a designated isolation site in the area. At the isolation site, the client would receive three meals a day, on-site medical treatment, necessities, and a safe place to stay during his recommended quarantine timeframe. As a result, the participant could obtain immediate services, followed by entry into the North County shelter program.

- Over the past month, Outreach Coordinators have consistently offered resources to an elderly individual facing street-level homelessness. The Outreach Coordinators collaborated with outside agencies and monitored the system that displays bed availability daily for any shelter openings. These efforts led them to find an opening at the Cold Weather Shelter in Santa Ana. The individual will now have on-site case management, essential necessities, and shelter.
- While responding to a call for assistance from a concerned resident, the Outreach Coordinators engaged a middle-aged male individual who has been experiencing chronic homelessness. The individual was exposed to a confirmed COVID-19 case and he was experiencing COVID-19 symptoms. Based on the information, the Outreach Coordinators began the process to connect the participant to a County-funded COVID-19 isolation site. Once the participant was evaluated, he was deemed appropriate for placement at one of their two isolation sites. At the isolation site, the client would receive three meals a day, on-site medical treatment, essential necessities, and a safe place to stay during his recommended quarantine timeframe. Equally important, the Outreach Coordinators are actively working with the client to put him into the Buena Park Navigation Center once his time in isolation concludes. The participant was able to obtain immediate services, which will be followed by an entry in the system of service provision.
- While responding to a call for assistance from the Orange County Sheriff's Department, the Outreach Coordinators engaged an elderly female inside the Sheriff's Substation. The individual was in dire need of shelter and housing resources. The individual was previously at the Salvation Army Emergency Shelter and was unable to determine if she qualified for re-entry. The Outreach Coordinator connected with the referring agency and verified the individual could return to their city shelter program. Transportation was provided to the individual and she is now at the Anaheim shelter program. The client will have supportive services that include housing, case management, and medical care.
- Public Safety staff has continued to address a variety of issues created by the homelessness issue. The attached photo shows a clean-up taking place and Garden Grove Blvd. and Court Avenue. The response shown involves the Outreach Coordinators, Public Safety, Orange County Sheriff’s Department and OC Public Health. This area has been problematic due to its proximity to the Garden Grove border and is subject to an influx of homeless individuals when there are enforcement efforts in neighboring cities.

- Stanton Outreach Coordinators consistently offered case management services to a transitional-aged participant (18yrs-26yrs). The individual was a Stanton resident who had recently become homeless and was in dire need of housing assistance. The Outreach Coordinators then collaborated with the Buena Park Navigation Center to connect the participant to shelter and additional resources. Additionally, the Outreach Coordinators assisted the participant with the process of connecting them to County’s Housing subsidies. As a result, the participant was matched to Orange County’s Rapid Rehousing Program, allowing the participant to begin their housing search. The Outreach Coordinators provided Housing Navigation to the participant to assist with locating a unit. The Outreach Coordinators were successfully able to locate an ideal unit for the participant in Anaheim, allowing them to remain local. The team also assisted the participant with transporting items from the shelter to his new home.
- While responding to a call for assistance from a concerned resident, the Outreach Coordinators engaged an elderly female participant who has been experiencing chronic homelessness. Based on the referral, the Outreach Coordinators began the process to connect the participant to County housing resources. Once the participant was evaluated, she was deemed appropriate for placement at the Buena Park Navigation Center. Additionally, the Outreach Coordinators completed a housing assessment and assisted the elderly complete the application process for county funded housing. As a result, the participate was able to obtain supportive services that include housing, case management, and medical care.
- Based on a referral from a non-profit agency, the Outreach Coordinators received a call for assistance regarding an elderly female in need of services. The individual was staying at a local Motel 6 and stated she could no longer afford the stay. The Outreach Coordinators immediately contacted the individual to go over possible resources. During the discussion, the Outreach Coordinators informed the individual of possible available resources and options for them. The individual expressed interest in seeking employment and finding an affordable place to live. The Outreach Coordinators worked with the participant and ultimately connected them with a third-party agency, People Ready, who aide individuals to find job placement. This referral allowed the client to find an employment opportunity in the City of Laguna Beach. Additionally, the Outreach Coordinators assisted the individual with finding an available housing option within the area of employment. This assistance led to the individual renting an RV in Laguna Beach while she begins her workforce journey. Lastly, the Outreach Coordinators connected the client to Mariners Church, and the church assisted them with groceries.

- Stanton Outreach Coordinators established and maintained contact with a transitional-aged participant (18-26 years old). The individual was a Stanton resident who had recently become homeless and was in dire need of housing assistance. The Outreach Coordinators collaborated with the Buena Park Navigation Center to place the participant in the shelter and connect the individual with additional resources. The Outreach Coordinators assisted the participant with the process of obtaining a housing subsidy from the County of Orange. As a result, the participant was matched to Orange County’s Rapid Rehousing Program which allowed the participant to begin their housing search. The Outreach Coordinators provided Housing Navigation services to the participant to assist with locating a suitable unit. The Outreach Coordinators were successfully able to locate an ideal unit for the participant in Anaheim which allowed the participant to remain local.
- While responding to a call for assistance from a concerned resident, the Outreach Coordinators engaged an elderly female participant who has been experiencing chronic homelessness. Based on the referral, the Outreach Coordinators began the process to connect the participant to County housing resources. Once the participant was evaluated, she was deemed appropriate for placement at the Buena Park Navigation Center. Equally important, the Outreach Coordinators completed a housing assessment and referred the elderly participant into the County community queue for Orange County funded housing. As a result, the participant was able to obtain supportive services that include housing, case management, and medical care.
- Stanton Outreach Coordinators responded to a call for assistance from a local business regarding two brothers experiencing homelessness. Upon arrival, Stanton Outreach Coordinators began assessing the individuals for services needed. The Outreach Coordinators immediately contacted Cal Optima to obtain physical care and mental health services for the brothers. The Outreach Coordinators connected the individuals to Cal Optima’s mobile crisis unit, operated by Central City Community Center. The individuals received wound care and were able to schedule future mental health appointments. In addition to receiving physical and mental health services, Stanton Outreach Coordinators did a warm handoff to Central City Community Center to refer the individuals to a recuperative care center. This referral allows for the most immediate access to health care services and shelter.

- Based on a referral from a non-profit agency, The Outreach Coordinators received a call for assistance regarding an elderly female in need of services. The individual was staying at Motel 6 and stated she could no longer afford the stay. The Outreach Coordinators immediately contacted the individual to discuss possible resources. The Outreach Coordinators discussed various resources and options for the elderly female. The individual expressed interest in finding employment as well as an affordable place to live. The Outreach Coordinators connected with People Ready, an organization that provides assistance to individuals in need of job placement. This referral allowed the client to find an employment opportunity in the city of Laguna Beach. Additionally, the Outreach Coordinators assisted the individual with finding an available housing option within the area of employment. - The Outreach Coordinators engaged a TAY (Transitional-Aged Youth) experiencing homelessness on the streets of Stanton. As a result of maintaining regular contact and establishing a rapport, it was discovered that the individual had just recently arrived in Stanton after previously residing in South Los Angeles. The individual inquired if he could be referred into a shelter and receive services in the South Los Angeles area. The Outreach Coordinators collaborated with the non-profit “The Good Seed,” located in Long Beach. This community partner was able to provide shelter to the individual, linkages to permanent supportive housing, and ongoing case management. Additionally, the Outreach Coordinators provided the individual with transportation to conduct a warm hand-off with the receiving agency.

- While responding to a call for assistance from OCSD on Monroe Avenue, Outreach Coordinators contacted a man currently residing in a makeshift shelter located on the sidewalk in front of Bethel Romanian Pentecostal Apostolic Church. It was determined that he met the criteria for Buena Park Navigation Center, and he was transported within one hour. Outreach Coordinator staff assisted with the clean-up of debris and tent removal.
- Based on a referral from a concerned business owner, the Outreach Coordinators received a call for assistance regarding an individual living in a vehicle that needed medical attention. The Outreach Coordinators immediately reached out to the Orange County Outreach and Engagement Team for collaboration and support. Through these efforts, the team requested a mobile medical unit to assess the participant. The participant was able to get wound care, prescription assistance, and future medical appointment assistance. The participant will continue to work with the Outreach Coordinator to obtain long-term medical treatment and housing support
- Over the past month, Outreach Coordinators have partnered with the OC Behavioral Health Liaisons, OCSD, and OC Public Works to conduct outreach and clean up the flood channels that run under the city. This operation is an ongoing collaborative effort to patrol the waterways, offer services, and ensure that encampments do not reoccur as the winter months approach. Keeping the flood channels clear of homeless individuals and the associated debris is a priority as the potential of rain events approaches. There are safety and environmental issues associated with debris in the flood control system.
OCTOBER 2021
- The Outreach Coordinators engaged two elderly participants experiencing homelessness at Stanton Park. Upon first contact, the Outreach Coordinators were able to provide emergency motel assistance to the participants, providing them with immediate shelter. Additionally, the Outreach Coordinators collaborated with Jamboree Housing to facilitate supportive housing services for the elderly individuals. This collaboration has led to the participants being housed at Stanton Inn where they will receive additional supportive services that include housing, case management, and medical care.
- Based on a referral from city staff, the Outreach Coordinators received a call for assistance regarding a severely disabled individual who needed medical aid. The Outreach Coordinators immediately began the evaluation process to provide the individual with connection to the county’s mobile medical team for immediate treatment. The mobile response team treated and transported the individual to a higher level of care. The Outreach Coordinators collaborated with a Recuperative Care Case Manager with the support of the medical response team to have the participant assessed by a physician. Once the participant was evaluated, he was deemed appropriate for recuperative placement. The participant is now receiving wrap-around services through a recuperative care agency.
SEPTEMBER 2021
- OCSD engaged an individual searching for shelter after recently becoming homeless in a neighboring city. OCSD was able to verify the participant’s ties to North Orange County, allowing the Outreach Coordinators to place the participant into the shelter the same day.
- Based on a referral from a concerned resident, the Outreach Coordinators received a call for assistance regarding a severely disabled individual who needed medical aid. The Outreach Coordinators immediately began the evaluation process to provide the individual with connectivity and transportation to a higher level of care. The Outreach Coordinators collaborated with a Recuperative Care Case Manager and Foothill Hospital to have the participant assessed by a physician. Once the participant was evaluated, he was deemed appropriate for recuperative placement. The participant is now receiving wrap-around services through a recuperative care agency.
- The Outreach Coordinators worked with a participant to obtain the required documentation needed for a permanent supportive housing application. As of September 8, 2021, the participant was matched to a housing voucher through the Orange County Coordinated Entry System. The Outreach Coordinators are currently submitting final documentation to the U.S Department of Housing and Urban Development for final evaluation.
- Based on a referral from OCSD, the Outreach Coordinators engaged an individual experiencing homelessness and in need of services. The individual had just recently become homeless after disconnecting from his family in Orange County. After engaging the individual and discussing potential solutions to the situation, it was discovered that the participant had a home to return to in South Carolina. After verifying that the participant had a place to return to, the Outreach Coordinators utilized the City of Stanton funds to help relocate the participant back to South Carolina. The Outreach Coordinators are currently taking steps to stabilize his situation and support from friends and family.
AUGUST 2021
- Over the past two weeks, Outreach Coordinators have assisted five individuals in applying for the County’s Rapid Rehousing Program. The process is currently coming to a close, and the individuals will officially enter the program within the month. This increases the total of program participants to eight.
- After contact from an elected official, the Outreach Coordinators were able to provide shelter at a motel for an individual who needed help. Outreach Coordinators are currently working on connecting the individual to additional supportive services. The individual has expressed a commitment to taking the necessary steps in exiting life on the streets permanently.
- Outreach Coordinators have partnered with the OC Behavioral Health Liaisons, OC Sheriffs, and OC Public Works to conduct outreach and clean up on the flood channels that run under the city over the past month. This operation is an ongoing collaborative effort to patrol the waterways, offer services, and ensure that encampments do not reestablish. It is important to continue this effort as it is an area of concern during the winter months. Measurable rain and debris in the flood channels can be problematic to the flood control system.

- Stanton’s Outreach Coordinators engaged an individual who had previously received services from various agencies but lost contact. The participant stated that he did not want to be reconnected with former agencies but instead wanted to work with the Outreach Coordinators to pursue permanent supportive housing. The Outreach Coordinators worked with the participant to obtain the required documentation needed for a permanent supportive housing application. As a result, the participant was successfully matched to a housing voucher through the Orange County Coordinated Entry System. The Outreach Coordinators are currently submitting final documentation to the U.S Department of Housing and Urban Development for final evaluation.
- The Outreach Coordinators assisted eight individuals apply and be selected for the County’s Rapid Rehousing Program. The County’s Rapid Rehousing Program provides participants with twelve months of rental assistance and housing sustainability services. In addition, the program provides the participants with housing navigation, security deposit assistance, and covers moving costs. All individuals have lived in the City of Stanton and have a history of falling into homelessness in Stanton. The Outreach Coordinators are currently conducting warm hand-offs between the participants and their new case managers.
- Stanton Outreach Coordinators received a referral from a concerned resident, requesting assistance for an individual living in his vehicle. The Outreach Coordinators engaged the regarding individual to inquire about services that may be needed. The Outreach Coordinators collaborated with the Orange County Outreach and Engagement team for additional support. Through these efforts, the team successfully requested a mobile medical unit to assess the participant and medical attention was provided. The participant received wound care, prescription assistance, and was able to schedule a future medical appointment. The participant will continue to work with the Outreach Coordinator to obtain long-term medical treatment and housing support.
JUNE 2021
- The Outreach Coordinators received a referral from a partnering agency regarding a family from out of state experiencing homelessness in a vehicle in the City of Stanton. The Outreach Coordinators engaged the family to provide resources and connection to family services. After further discussion, it was found that the family was originally from Louisiana and had ties to Los Angeles County in the past. The Outreach Coordinators deemed it appropriate to bridge the family into a motel until connection to family services were obtained in Los Angeles County. The City of Stanton provided the family with a motel stay and meal cards during the transitional period between services from one agency to another. The Outreach Coordinators were able to connect the family to Los Angeles Family Housing, which will provide ongoing case management, shelter, and housing navigation.
- The Outreach Coordinators engaged an individual experiencing homelessness located at the corner of Katella and Beach Blvd. The individual was a disabled 59-year-old male who was in need of medical attention. The Outreach Coordinators requested medical assistance to provide support to this individual’s needs. The Outreach Coordinators transported the participant to the Foothill Hospital’s Emergency Room to be assessed by a physician who collaborates with our Outreach Coordinators. After the participant was assessed, he was deemed appropriate to be placed into a recuperative care center for further support and ongoing medical assistance.
- The Outreach Coordinators received a referral from OC Behavioral Health in regards to a participant who was interested in the shelter. Upon arrival, the Outreach coordinators engaged the individual and assessed the participant for ties to the North SPA. Once ties were confirmed the Outreach Coordinators were able to refer the participant to the Buena Park Navigation Center where he will be connected to housing resources and navigation. The Outreach Coordinators also assisted their participants with employment connectivity. The participant was onboarded to a full-time job located in Anaheim, CA. The participant continues to reside at the shelter and has assistance from the City of Stanton’s Outreach Coordinators for transportation and support.
MAY 2021
- A former Stanton homeowner that had fallen into homelessness at the age of 75 and was placed into a transitional living situation through rapid rehousing funding provided by the City. This action helped prevent the individual from falling into street-level homelessness.
- A veteran living in the city was evicted from his apartment due to insufficient funding to pay rent. The individual went to City Hall and was connected with Damian. Damian made contact with the Veteran Intake Coordinator from the County of Orange and successfully connected the individual with existing resources which led to a brief motel stay before being connected to resources leading to permanent housing.
- A 74-year-old male that had fallen into homelessness one week earlier, made contact with City staff at the Family Resource Center. He was connected to the Outreach Coordinator and was able to receive services which led to his exit from the streets within a day.
- The Outreach Coordinator was able to assist a member of the homeless community to receive his COVID-19 vaccination by transporting him to the Anaheim Convention Center.
FEBRUARY 2021
- The Outreach Coordinator engaged a disabled couple at Stanton Park; both individuals were in wheelchairs and expressed interest in shelter. Due to the circumstances of not being able to transport the participants then and no shelter available, the Outreach Coordinator reached out to City Net and OC Behavioral Health to support and coordinate services. The Outreach Coordinator decided to bridge the couple into a motel while City Net provided the transportation to get them there and provided meal cards for the duration of their stay. The Outreach Coordinator coordinated with OC Behavioral Health to refer the participants to the Yale Navigation Center since they had strong ties to Santa Ana. On March 3, 2021, the Outreach Coordinator and OC Behavioral Health Personal accompanied the couple to the Yale Navigation Center to conduct a warm hand-off.
DECEMBER 2020
- The Outreach Coordinator received a referral from a Community Services staff member about a family searching for homeless services. The Outreach Coordinator reached out to the family to evaluate the family’s situation and needs. The family of four had been placed into a motel by 211OC but were coming to the end of their stay and needed further assistance to avoid spending the night on the street. Although the family was based out of Buena Park, the Outreach Coordinator coordinated and obtained emergency assistance from the City Net and Pathways of Hope, bridging the family into a hotel until the appropriate family shelter was available.
- The Outreach Coordinator received a call from OCSD personnel seeking collaborative assistance with the CAT (Crisis Assessment Team). The team searched for a shelter bed since the participant did not meet the criteria to be 50/51 at the time of engagement. The Outreach Coordinator was able to assist with locating an appropriate shelter bed with OC Behavioral Health Services' help. The Outreach Coordinator coordinated a ride and accompanied the participant to ensure the participant arrived at the county's Behavioral Health Services office for further evaluation and connection to supportive services.
NOVEMBER 2020
- The Outreach Coordinator engaged an individual experiencing homelessness on the railroad tracks. The participant was in search of food and shelter but, most importantly, inquired about medical treatment. The participant stated that he had a leg injury and was in severe pain. The Outreach Coordinator coordinated transportation for the participant to be taken to St. Jude Medical Center; the Outreach Coordinator accompanied the participant to the medical center to ensure the participant successfully sought out treatment. Due to Covid-19 regulations, the Outreach Coordinator could not enter the medical facility with the participant. The participant will connect with the hospital's social worker for aftercare or shelter options.
- The Outreach Coordinator received a referral from a Community Service Staff member about an individual experiencing homelessness outside of the Stanton Family Resource Center. The individual was a TAY (Transitional-Aged Youth) who had just recently became homeless due to loss of job stability and unstable family dynamics. The Outreach Coordinator referred the participant to the Buena Park Navigation Center, where an intake assessment was conducted along with a bed assignment. The Outreach Coordinator also assisted the participant with filling out a job application; the following week, the participant informed the Outreach Coordinator that he is now employed.

A member of the homeless community who has refused services on several occasions was sleeping outside of a storefront. Initially, the participant was reluctant to participate, based on prior experiences. The Outreach Coordinator engaged the individual and provided information regarding the services available, including the coordination of an appointment at a local methadone clinic. The Outreach Coordinator was able to complete an assessment with the participant, secure the methadone clinic appointment, and get her into the Buena Park Navigation Center the same day.
